Preventive Care Reminders

Promote preventive care with reminder calls

Improve your health management program by conducting preventive care reminders during both inbound and outbound calls. SironaHealth supports your population health management programs through the delivery of preventive care reminders or health referrals.

For example, our preventive care reminders can:

  • Remind diabetic patients of the importance of annual eye exams. For callers without an optometrist or endocrinologist, we can help them find and schedule appropriate exams with in-network specialists.

  • Suggest to consumers over 50 that they should get their seasonal flu shot, providing them with directions to your available flu clinics or helping them schedule a visit with their primary care provider.

What's the best way to conduct care reminders?

The best way to remind your patients to stay healthy, and to connect them with relevant health resources, is to engage them at multiple touch points.

We'll help you engage your patients or health plan members with a variety of outbound calling services along with targeted messaging for callers who opt to dial-in to speak with a registered nurse or other healthcare representative.

Our preventive care reminders include:

  • Outbound reminders - Registered Nurses (RN) or Medical Service Representatives (MSRs) make outbound reminder calls.

  • Inbound reminders - During a Nurse Advice Line call, our system reminds the nurse to conduct a preventive care reminder based on the caller's current health status, demographics, and/or geographic location.

  • Automated calls - Schedule automated reminders utilizing interactive voice services, which can escalate to an RN at anytime.

As part of each preventive care reminder, we can also assist your callers in making an appointment. Whether that means giving your caller a phone number, conducting a warm transfer, or working directly with your scheduling system.

Information on which care reminders are delivered is automatically provided to you and/or to the caller's Primary Care Provider for appropriate follow-up.
